Allow manual declaration of incident start and end times #500

Many incidents occur without a monitor directly attached to the component which fails. We often have to adjust incident start and end times to accurately reflect the true incident scope and duration. Currently it is impossible for us to use the declare incident feature as it always assumes that the incident begins at the exact time that the form is submitted.

Please add manual entry of the start time to the declare incident workflow.

Please add manual entry of the end time to the resolve incident workflow.

One of the best things about OneUptime is essentially useless to us until this is implemented.
