Explain in detail what your suggested feature would be used for.

While in an instance, any new avatars that load will be recorded and saved to the log along with the avatar ID. If someone changes avatar and your game loads it, it gets logged along with the avatar ID. If you join a new instance, when your game loads someone, their current avatar along with the ID goes in the log.

Describe how it would look if it requires a UI.

It would go in the game log section of the UI. Like when a friend changes avatar, it would display the username of the player then the avatar name/ID of the avatar they swapped to/were when you joined. It should be clickable like avatars listed when friends swap.

Explain why people would want to use it.

If someone has a malicious avatar, often by the time they use it, it's too late to bring up the menu for the avatar and report it. Saving every avatar's ID would allow someone to go back in their logs, locate the malicious avatar that was loaded, locate the ID of the avatar and report it. As of current, crasher avatars have no real way of being reported, as by the time you realise, the game has closed or you never even saw it with your eyes. Yes there's no way of recording which avatar was malicious but knowing the time and ID, it wouldn't be hard to work out.

It could also be used with the charts feature of vrcx. If public avatars are used by multiple people, you can chart how often you see a specific avatar and data around avatars.
